Remote patient monitoring uses digital technologies combined with St. Vincent care managers to monitor and collect medical and health data from patients for the purpose of improving health outcomes.
Our Care managers includes RN, Pharmacist, and Pharmacy Tech. They will collect a wide range of health data from patients enrolled in monitoring programs, including vital signs, heart rate, blood pressure, weight, blood sugar, blood oxygen levels, and even electrocardiograms.
Also providing remote patient monitoring can proactively identify emerging issues before they become full-blown problems and provide immediate interventions – leading to improved outcome measures, reduced hospitalizations, and reduced E.D. visits.
Applications for Remote Patient Monitoring
Where remote patient monitoring is most valuable is for patients with one or more chronic conditions, such as congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease, diabetes, hypertension, and asthma. In addition to monitoring biometric readings for these patients and intervening when necessary, our care managers can:
● Evaluate patients’ medication compliance, including whether a patient has filled a prescription on time or is taking a drug as prescribed.
● Help patients identify the link between the onset of symptoms and a newly prescribed drug.
● Provide education to help patients understand their co-morbidities and how best to manage their conditions.
● Coach patients to better manage their overall health.
● Assist clinicians with monitoring a patient’s treatment plan.
● Help patients get the most from their home health visits

Service Initiation – billed under CPT 99453 – $19 (Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate), initial; set-up and patient education on use of equipment).
Data Transmission – billed under CPT 99454 – $62.80 (Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate), initial; each 30 days).
Data Analysis and Interpretation – billed under CPT 99091 – $52.87(Collection and interpretation of physiologic data (e.g., ECG, blood pressure, glucose monitoring) digitally stored and/or transmitted by the patient and/or caregiver to the physician or other qualified health care professional, qualified by education, training, licensure/regulation (when applicable) requiring a minimum of 30 minutes of time, each 30 days).
Treatment Management Services – billed under CPT 99457 – $54.94(Remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month initial 20 minutes) and CPT 99458 – $42 (additional 20 minutes)
